The U.S. Food and Drug Administration today approved a new higher dose (7.2 mg) of Wegovy (semaglutide) injection, called Wegovy HD, for weight loss and long-term maintenance of weight loss for certain adult patients. The decision was granted just 54 days after filing and represented the fourth approval under the Commissioner's National Priority Voucher (CNPV) pilot program.

Impressive Weight Loss Results

Clinical trials reveal remarkable effectiveness for the higher-dose formulation. The high-dose Wegovy helped patients with obesity lose an average 20.7% of their weight after 72 weeks in a phase three trial. Called Wegovy HD, the higher-dose drug helped participants in a study lose about 19% of their body weight, or almost 47 pounds, versus about 16% of body weight, or 39 pounds with the lower dose over almost 17 months.

The results extend beyond simple weight loss numbers. For those taking Wegovy HD, 89% achieved ≥ 5% body weight loss versus 38% taking placebo (from a baseline body weight of 248 lbs). Even patients with diabetes, who typically face greater weight loss challenges, saw substantial benefits. In a separate phase three trial on patients with obesity and Type 2 diabetes, high-dose Wegovy demonstrated an average weight loss of 14.1%.

Safety Profile and Side Effects

While the higher dose delivers enhanced results, it comes with increased side effects. Side effects such as nausea, vomiting and constipation were reported in more than 70% of those who took the higher dose of Wegovy, compared to more than 60% taking the lower dose and about 43% taking a dummy medication, the study showed. A notable concern involves altered skin sensations. Events related to a clinical picture of altered skin sensation such as sensitive skin, hyperesthesia, dysesthesia and paresthesia were reported by a higher proportion of participants, and at a higher rate, in the Wegovy® HD arm (22%), compared to treatment with Wegovy® 2.4 mg (6%) and placebo (0.3%).

Healthcare experts acknowledge both the promise and precautions surrounding the new dose. Dr. Jody Dushay, an endocrinologist and obesity expert at Harvard Medical School, welcomed the approval of the higher-dose shot. It "may be especially helpful for people who are tolerating the lower-dose version, but "have had suboptimal weight loss," she said in an email.

Market Impact and Availability

Novo Nordisk positions this higher dose strategically against competitors. The Danish drugmaker is positioning that version to better compete with Lilly's obesity drug Zepbound, which has proven to be more effective at promoting weight loss. Novo expects to launch the higher, 7.2-milligram dose of Wegovy in April.
